Page 4 of 4 6. RECORD KEEPING AND ACCIDENT REPORTING PROCEDURE 6.1 All injuries or illnesses that result in first aid being administered must be recorded on a First Aid Treatment Card. The treatment card should be kept with the first aid officer/appointed first aid officer or stored in a secure place not accessible by others. 6.2 All accidents must be recorded using the accident report form and a copy given to the person’s line manager and the Human Resources Manager. Major injuries or dangerous occurrences must be reported immediately to the Human Resources Manager. 7. TRANSPORTATION OF CASUALTIES 7.1 It is the responsibility of the First Aid Officers to determine who needs emergency / hospital care. All people with serious injuries and major illnesses should be sent to hospital immediately via ambulance. 8. FIRST–AID RESOURCES 8.1 Requirements At least one first aid container is required at a work-site and must be sufficiently stocked with suitable quantities of first aid materials, according to the risk posed in the working environment. First aid notices must be displayed clearly with the name of the appointed first aid officer visible. Tablets and other medication should never be kept in a first aid container. It is the responsibility of the appointed first aid officer to maintain the supplies of the first aid container.
