IMMEDIATE RESPONSE TO ACCIDENT / NEAR MISS 5.1 It should be determined whether prompt and appropriate action was taken:  Fire fighting.  First aid.  Area made safe.  Access restricted.  Electricity isolated.  Warning notices displayed. 6. IDENTIFY PREVENTATIVE MEASURES 6.1 Preventative measures should be identified including:  Was there an up to date risk assessment carried out for the task?  Review the risk assessment for the task.  What safety precautions were in place and what safety precautions should have been in place?  Was instruction and training appropriate to the task? 7. IDENTIFY UNDERLYING CAUSES 7.1 The underlying causes should be identified:  Was supervision and training adequate?  Was equipment suitable for the task?  Was equipment maintained and tested adequately?  What pressures / constraints, if any, were being applied?  Was communication adequate between relevant parties? 8. ACTIONS TO PREVENT A RECURRENCE 8.1 The actions required to prevent a recurrence of the accident should be considered.  Could the outcome have been more serious?  What needs to be done to prevent similar accidents / near misses? Why weren’t such measure implemented previously?  Were the safety precautions adequate?
