Page 1 of 2 ACCIDENT REPORTING PROCEDURE 1. INTRODUCTION 1.1 The aim of this procedure is to provide a clear framework for reporting accidents. 1.2 The policy takes into account the Health and Safety at Work Act 1974, RIDDOR - Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013 and the General Data Protection Regulation 2016. 2. WHAT SHOULD BE REPORTED? 2.1 In the event of any of the occurrences shown below, please complete an Accident Report Form and return it to the Human Resources Manager. A photocopy of this form should be sent to the line manager of the injured person. In order to comply with the General Data Protection Regulation 2016, personal details entered in accident forms must be kept away from public view. All copies should be kept in a secure location (scanned into the ‘Accident or Near Miss Report’ screen on the HR System). This procedure includes contractors, visitors and members of the public who do not have line managers on site. All accidents should be reported to the Human Resources Manager immediately but those affecting contractors should also be reported to their employer as soon as possible. 2.2 Please report any of the following occurrences on the accident report form: Any accident regardless of severity or activity at time of the occurrence. Injury to person or persons. Near misses - any occurrence where an accident/injury has been narrowly avoided. Death of person or persons due to an accident. Physical abuse. Verbal abuse. Threatening behaviour. 2.3 In all cases, if the injury / near miss was caused by faulty equipment / damaged property then the Property Services Manager must be informed immediately so that repairs can be made. 2.4 WATMOS is responsible for reporting major injuries and incidents to the Health and Safety Executive (HSE) under the Reportable Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR). It is therefore vital that the following should be reported to the Human Resources Manager immediately: The death of any person as a result of an accident arising out of or in conjunction with work. Major injuries including any fracture, other than to the fingers, thumbs or toes, any amputation, dislocation of the shoulder, hip, knee or spine, loss of sight (whether temporary or permanent), a chemical or hot metal burn to the eye or any penetrating injury to the eye, any injury resulting from an electric shock or
